Which forms do I require?

• Fieldwork risk assessment form  (Google Docs) - for field work and field trips (except survey-based fieldwork)
• General risk assessment form (MS Word) - for laboratory work and survey-based field work
• CoSHH assessment form - an additional assessment required for lab or field work involving chemicals. You will be prompted to make a copy to save an editable version of this document to your Google Drive, complete it and then share it with or send it to environment-safety@york.ac.uk
• Student laboratory user agreement
